Staff Forms

  • Registration Form - This form is for hired staff (including faculty) only.
  • Liability Waiver - Return to the Admin Department or
  • Emergency Form - Return to the Admin Department or
  • Expense Reimbursement Form - to be used when turning receipts in to the Accounting Office
  • Leave Request Form - For full-time year round employees only; return to the Director of Finance and Operations
  • Time sheets - For hourly employees only. Payroll work periods are bi-monthly (twice a month) and are from the 12th-26th and the 27th through the 11th of the following month. (The work period for salaried employees remains the 1st-15th and 16th to the last day of each month.) Please Note: Time sheets are due to your supervisor by 5:00pm the 11th and 26th of each month, or before. Ask your supervisor about possible electronic submission of hours to the Accounting Office.
  • Time and Effort Reports - For all salaried positions paid through NSF grants.
